**各会员单位：**
由广东省定制家居协会和广东衣柜行业协会主办的第二届“华立股份定制杯”篮球联赛已持续一个多月，即将进入尾声。届时男子公益明星赛、女子篮球赛将与总决赛同台呈现，为下一期梦想读书站慈善活动进行募捐，比赛时间地点待定，相关比赛细则、报名事项如下：
**一、男子公益明星赛**
男子公益明星赛由两部分内容组成：篮球明星赛+公益款项捐献，现公开招募定制家居行业公益明星赛球员，报名人员要求为各企业董事长或副总及以上职位的人员，共招募24名球员。
本次男子公益明星赛所筹集款项（报名费），除去活动筹办经费，其余将全部捐助“梦想读书站”第6期项目，用于当地贫困小学建设图书室，传递知识，传递爱心。协会现号召各企业踊跃报名，展现广东省定制家居协会和广东衣柜行业协会“公益+健康”的风采，体现会员企业的善举魅力！
**（一）竞赛办法**
24名队员，现场抽签分为两队，每队12名运动员，共一场比赛。
**（二）奖项设置**
1.冠军队：奖杯+精美礼品
2.亚军队：奖杯+精美礼品
**（三）报名办法**
1.报名人员要求为定制家居行业企业董事长或副总及以上职位的人员，共招募24名球员，先报先得。
2.球衣将由华立股份统一赞助，请各球员将**球衣号码及码数**报到秘书处。
3.参赛队员报名费：**5000元/人**，如需要发票的（另加6个税点）。
付款方式： 7个工作日内付款，款项请汇入以下账号：

**二、女子篮球赛**
**（一）竞赛办法**
24名队员，现场抽签分为两队，每队12名运动员，共一场比赛。
**（二）奖项设置**
1.冠军队：奖杯+精美礼品
2.亚军队：奖杯+精美礼品
**（三）报名办法**
1\.**定制家居行业企业、媒体女性正式员工均可免费报名参加**，不限职位，共招募24名球员，先报先得。
2.球衣将由华立股份统一赞助，请各球员将**球衣号码及码数**报到秘书处。
**三、注意事项**
（一）参赛运动员必须身体健康，患有心脏病、高血压症、哮喘病等不适宜参加剧烈运动疾病者禁止参赛。
（二）本着“友谊第一、比赛第二、促进交流、共同进步”的宗旨进行比赛，各球队注意安全教育，禁止发生不和谐事件。
（三）建议各参赛运动员自行购买 “人生意外伤害保险”（含往返比赛场地途中及比赛期间）。
（四）各参赛运动员交通费用自理。
**四、报名方式**
各运动员请于2017年8月18日前向协会秘书处上报报名信息及其他资料（见附件1，附件2）。 附件报名信息填写后发送于协会邮箱2691383329@qq.com。
